NorthAble's Transition Service

NorthAble's Transition Service will work with Ongoing Resourcing Scheme (ORS) funded students as they transition out of education to create a plan that focuses on their wants, needs, aims and aspirations.

Working with you, your whānau and your school, we will look at what you want to achieve in your life after school and what steps or supports may be required to enable you to achieve this. 

We can work with you to create opportunities in everything that matters to you, from jobs and training to your hobbies, interests, and activities. Because we work across Te Tai Tokerau, we can meet you somewhere you feel comfortable and as often as you need us to. 

We know what you want to achieve is personal to you, so we will work with you personally to help you be all you want and can be. 

Our Process

  1. When you plan to finish school, we will talk to you about options for Transition.
  2. If you choose NorthAble for Transition, your Service Facilitator will meet you to discuss your goals.
  3. A plan is drawn up with you that outlines what we are doing and how. 
  4. We work with you and other providers, such as your school and WINZ, to bring the plan to life. 
  5. We work together to track how everything is working out, changing things as needed.
  6. When it is time to leave school all the foundations will be in place to make the transiton as easy as possible. 
  7. Your Service Facilitator will continue to support you once you have left school and can make sure everything is going well.
